چکیده

Lung hyperinflation commonly accompanies expiratory flow limitation in patients with chronic obstructive pulmonary disease (COPD) and contributes importantly to morbidity and an impoverished quality of life. It is not surprising, therefore, that lung hyperinflation has become an important therapeutic target in symptomatic COPD patients. Acute dynamic increases in lung hyperinflation under conditions of worsening expiratory flow limitation and increased ventilatory demand (or both) can seriously stress cardiopulmonary reserves in patients with more advanced disease. The present understanding of the physiological mechanisms of dynamic hyperinflation during exacerbations of COPD and during physical activity continues to grow, together with an appreciation of its negative mechanical and sensory consequences. In this brief overview, definitions and measurement of lung hyperinflation during rest and exercise will be discussed and its potential clinical importance will be considered. The focus will mainly be on current concepts of the mechanisms of air trapping and its role in inducing respiratory discomfort and activity

COPD: Journal of Chronic Obstructive Pulmonary Disease

Lung hyperinflation commonly accompanies expiratory flow-limitation in patients with Chronic Obstructive Pulmonary Disease (COPD) and contributes importantly to dyspnea and activity limitation. There is increasing evidence that acute dynamic increases in lung hyperi...

Lung hyperinflation in COPD: applying physiology to clinical practice

In chronic obstructive pulmonary disease (COPD), worsening expiratory flow limitation together with alteration in the elastic properties of the lung are associated with progressive lung hyperinflation and gradual decline in the resting inspiratory capacity over time. Dynamic hyperinflation (DH) refers to the variable increase in end-expiratory lung volume (EELV) above the relaxation volume (VR)...

Early COPD patients with lung hyperinflation associated with poorer lung function but better bronchodilator responsiveness

BACKGROUND It is unknown whether aggressive medication strategies should be used for early COPD with or without lung hyperinflation. We aimed to explore the characteristics and bronchodilator responsiveness of early COPD patients (stages I and II) with/without lung hyperinflation. METHODS Four hundred and six patients with COPD who performed both lung volume and bronchodilation tests were ret...

LUNG HYPERINFLATION IN COPD: What the Clinician Needs to Know

Lung hyperinflation, chronic dyspnea and reduced exercise capacity are inextricably linked and are independent predictors of increased mortality in COPD. It is no surprise, therefore, that a major goal of management is to reduce lung hyperinflation in the hope of improving dyspnea, exercise tolerance and related long term patientcentered outcomes. The negative effects of lung hyperinflation on ...
